Criteria for Referral to the Scheme
The family should have at least one child from conception to 12 years
The family must live in the County Borough of Wrexham
The family must be experiencing difficulties
The family must agree to the referral
Who may make a Referral?
The scheme operates an open referral policy. This means anybody can make a referral to the scheme. Examples may include a range of professionals such as Health Visitors, Social Workers and Midwives, and Voluntary Organisations including Barnardo’s, Action for Children and Women’s Aid, plus Friends, Family, and Mum’s and Dad’s can also refer themselves.
How to Make a Referral
The family should be given a Home-Start Leaflet prior to completing the form. (Further copies of leaflet are available on request from the office in a variety of languages)
All referrals must be made on a Referral Form
It is important to complete all parts of the form.
What Happens to the Referral Next?
All referrals will be acknowledged by post or email within five working days from receipt of referral. If an acknowledgment is not received within this time please contact the Home-Start office to check we have received it.
If there is a waiting period we will notify both the family and referrer of anticipated time-scales.
Following an Organiser’s holistic assessment of a family’s needs the referrer will be contacted in the following way:
Phone contact from an Organiser to explain the process
Written contact to inform of match with volunteer or return of referral
