Who can get support?
The family should;
- have at least one child from conception to 12 years
- live in the County Borough of Wrexham
- must agree to the referral or can refer themselves
Who may make a Referral?
The scheme operates an open referral policy. This means anybody can make a referral to the scheme. Examples may include a range of professionals such as Health Visitors, Social Workers and Midwives, and Voluntary Organisations including Barnardo’s, Action for Children and Women’s Aid, plus Friends, Family, and families can also refer themselves by calling 01978 366660 or emailing homestartwrexham@gmail.com/ whatsapp 07936583987
How to Make a Referral
The family should be given a Home-Start Leaflet prior to completing the form.

All referrals must be made on a Referral Form
It is important to complete all parts of the form.
What Happens to the Referral Next?
All referrals will be acknowledged by post or email within five working days from receipt of referral. If an acknowledgment is not received within this time please contact the Home-Start office to check we have received it.
If there is a waiting period we will notify both the family and referrer of anticipated time-scales.
